Medical Information & Insurance


It is the parent’s responsibility to inform the coach of any known health problems or restrictions of their child.

Medical Release Forms 

A signed medical release form is mandatory for each child playing AYSA soccer. The medical questionnaire/release form must be completed and signed by the parent/legal guardian for each child before the child can play or practice. This form was on the back of the registration form you filled out. A copy of this form is given to your child's coach, or can be found on the back of the Registration form on the Forms page. 

Soccer Association for Youth (SAY) Accident Insurance 

Currently, part of your soccer registration fee pays for secondary accident insurance coverage while your child is participating in the AYSA program. Details of coverage can be obtained from any board member. In the event of an injury a SAY Injury Report must be completed immediately. This form can be obtained from the coach.
